I understand that in order to be eligible for the Flint Promise I must be a City of Flint resident and that I must graduate from a City of Flint high school before the end of this school year.

I hereby authorize any of the participating institutions through its agents and employees to release any financial aid, demographic, or academic information to the Flint Promise Flint & Genesee Group, and Flint & Genesee Education and Talent for the purpose of facilitating their analysis of my scholarship eligibility and the educational and economic impact of this scholarship on the City of Flint Area.

This release is authorized for the duration of the time that I receive scholarship funds. In order to rescind this Authorization, I acknowledge that I must submit written notification of rescission to the Office of the Registrar at my school of choice. This rescission would result in the termination of my receipt of the scholarship.

I also give permission to the Flint Promise Flint & Genesee Group, and Flint & Genesee Education and Talent to use information regarding my scholarship in promotion of the Flint Promise.

By selecting “I Agree”, below, I am indicating that I have read this Authorization to Release Educational Records and that I authorize the release of records as described above.

Selecting “I Agree” also indicates that I have read and agree to adhere to the scholarship conditions as set forth on the Flint Promise website, including graduating from an eligible high school, successfully completing the FAFSA, and successfully applying to participating institutions. Additionally, I acknowledge that the Flint Promise will meet the costs of tuition and fees alone, and will only do so after Pell grants and TIP grants have been applied at participating institutions.  Furthermore, I understand that I must maintain full-time status as a student or complete 24 credits in a school year, and maintain satisfactory academic progress and satisfactory academic standing as defined by Pell grant regulations and the participating institution of my choice.

I understand that failure to adhere to these requirements may result in the termination of my receipt of this scholarship.

I understand that in order to be eligible for the Flint Promise, at the time of application I must be a City of Flint, Michigan resident, that I must remain a City of Flint, Michigan resident from the time of application until my graduation from a City of Flint high school and that I must graduate from a City of Flint high school before the end of the school year in which I apply for the Flint Promise Scholarship.

By agreeing to Release of Information, I authorize any of the schools which participate in the Flint Promise Scholarship, through their respective agents and employees including the school I currently attend or any school I have attended in the past, whether that school participates in the Flint Promise Scholarship or not, to release any financial, demographic, academic  or personal information requested to the Flint Promise Scholarship Fund, to the Flint & Genesee Group, and Flint & Genesee Education and Talent for the purpose of determining my scholarship eligibility and to facilitate their analysis of the educational and economic impact of this scholarship on the Flint area.

I grant permission to the Flint Promise Scholarship Fund, to the Flint & Genesee Group, and Flint & Genesee Education and Talent to use any photograph(s) or electronic or digital images of me or including me in any social, print, electronic or digital media and in any presentations or promotions of any kind whatsoever.  Images will be stored in a secure location and only authorized staff will have access to them. They will be kept as long as they are relevant and after that time destroyed or archived.

This Release and Authorization shall remain in force for the duration of the time that I receive funds from the Flint Promise Scholarship Fund.  To rescind this Release and Authorization, I acknowledge that I must submit written notice of rescission to the Flint Promise Scholarship Fund and to the Office of the Registrar at the college I am enrolled with.  This written notice must be delivered personally or by first class mail return receipt requested and received or electronically by receipt of a read receipt.  Rescission of this Release and Authorization will result in my immediate ineligibility to receive Flint Promise Scholarship Funds, termination of payment of Flint Promise Scholarship Funds for  my benefit and make me ineligible to receive Flint Promise Scholarship Funds in the future.

By selecting “I Agree,” I state that I have read this Authorization for use of and Release of Information and that I authorize the use of and release of Information described herein.

By selecting “I Agree”, I state that I have read and agree to adhere to the scholarship eligibility conditions as set forth on the Flint Promise Scholarship Fund website, as they may change from time to time.  At the time I have selected “I agree” those scholarship eligibility conditions include, but are not limited to: graduating from an eligible City of Flint high school; successfully completing and supplying to the Flint Promise Scholarship Fund the FAFSA (Free Application for Federal Student Aid); and successfully applying to and being accepted at a Participating Institution.

If I am determined to be eligible to receive funds from the Flint Promise Scholarship Fund, I acknowledge and understand that the Flint Promise Scholarship Fund will pay the costs of tuition and fees directly to the Participating Institution after the last day to drop classes, that it is a “last dollar scholarship” that will only be applied after all other scholarships which I may receive and after all Pell grants and TIP grants have been applied at the Participating Institution.  Furthermore, to remain eligible for receipt of funds from the Flint Promise Scholarship Fund, I understand that I must maintain full-time status as a student by successfully completing 24 credits in each school year, and make satisfactory academic progress and maintain satisfactory academic standing as defined by Pell grant regulations and the Participating Institution in which I am enrolled.

I understand that failure to adhere to the requirements of the Flint Promise Scholarship Fund will result in the termination of my eligibility for receipt of this scholarship.
